Support Coordination
Support to help you understand your NDIS plan, connect and manage services, and achieve your goals.
What is NDIS Support Coordination?
Support Coordination helps people with disability navigate the NDIS. Our team of experienced Support Coordinators work closely with participants and families to ensure each participant is living their best life. This is achieved by building capacity and confidence in managing their NDIS plans, understanding their unique goals and support options.
Support Tailored Together
Our Support Coordination team covers a wide range of areas to ensure that you have access to the support you need at every step of your journey. Some of these key areas include:
We assist you in understanding and implementing your NDIS plan, ensuring that you have a clear understanding of your budget and the supports available to you.
We help you navigate the vast array of service providers, ensuring you have access to quality services that align with your goals and preferences.
Our Support Coordinators work collaboratively with you to identify your goals, develop a plan, and connect you with the right supports to achieve them.
We are your voice, supporting you to exercise choice and control over your life. We empower you to make informed decisions and access the services and resources you need.
We regularly monitor your progress, review your goals, and ensure that your NDIS plan remains aligned with your evolving needs and aspirations.
Stories from our community

Our community services are for people with disability who want support to live more independently, build skills, stay connected, and enjoy life in their community. We tailor supports around what matters most to you, your goals, interests, and pace.
Most of our services are funded through the NDIS. If you are unsure whether your plan covers a service, our team can help you understand your funding or connect you with Support Coordination to explore your options.
That’s okay. Many NDIS plans are flexible. If your goals align with building independence, community participation, or wellbeing, you may still be able to use your funding. Our team or a Support Coordinator can help you understand your plan.
Yes. Everything we do is person-centered. We work with you to understand your goals, whether that is building daily living skills, making friends, staying active, travelling, or living more independently and tailoring those supports around what works best for you.
Yes. Families and carers are welcome to be involved in planning, goal setting, and communication as much or as little as the participant chooses. We value collaborative support.
Absolutely! Many people use a mix of services. For example, Day Programs alongside In Home and Community Support, or Support Coordination while living in one of our Independent Living Homes.
We focus on supporting and advocating for those we provide services for. We are all about choice, and genuine connections. Our services are shaped around you and we’re not a one-size-fits-all model. We prioritise respect, consistency, and meaningful outcomes that support independence and belonging.
